Northland Future Health Care Planning Governance Group (formerly Northland ACP Steering Group)
- The Northland ACP Steering Group was established in 2011/12 to provide oversight of the ACP programme and ensure the implementation of ACP goals and objectives across Northland.
- The group was refreshed in late 2016 and renamed the Northland Future Health Care Planning Governance Group to incorporate the continuum of care – advance care planning, end of life care, and last days of care.
- The Governance Group is led by Northland’s Acting Director of Nursing and Midwifery.
- The group meets once every two months.
The Northland Future Health Care Planning (FCP) Governance Group provides strategic oversight of work in Northland pertaining to health care planning focusing on advance care planning (ACP), end of life care (EOLC), last days of life, and resuscitation treatment decisions.
The vision of the FCP Governance Group is the development of a culture that is patient and whanau centric, where patients and families are empowered to articulate decisions about their health care. This is supported by clinicians who are confident to engage and document patient priorities with an overarching commitment to the principles of Te Tiriti O Waitangi. A significant focus is related to ACP.
