Health Integration Projects (HIP) aim to integrate the care of patients across primary (secondary) and community based health care to improve patient experience; access to the ‘right care, at the right time from the right provider(s) and so improve health outcomes and system efficiencies. At the centre of this work is the principle of providing people centred care, close to home. In keeping with the ‘best practice’ approach to complex care integration, the HIPs goals are defined within four interlinked projects, namely:

  • providing people centred care which is close to home and maximises the use of health, community-based and social service providers (Care Coordination Networks);

HIP represents a collaboration between primary and secondary services, the Northland District Health Board and Mahitahi Hauora Primary Health Entity.
